In many cases, it’s a clinically recognized phenomenon called anosognosia—a neurological impairment in self-awareness that affects up to 50% of individuals with serious mental illness, including bipolar disorder and schizophrenia.Reference

People experiencing anosognosia genuinely do not perceive that they are unwell, making traditional approaches like reasoning, pleading, or forcing help ineffective and often harmful to relationships. Yet families are rarely taught this.
