Where is the Team located?
The Life Adjustment Team has two convenient locations in the Los Angeles area. The first is conveniently located at 11936 W. Jefferson
Boulevard, Suite B, Culver City, CA 90230, close to West L.A., the
South Bay, Santa Monica and LAX. We can be easily reached by exiting
the 405 Freeway at Jefferson and heading west. The office is located
approximately ½ mile West of the 405 Freeway on the South
side of the street.
Our other location is at 1260 Huntington Drive in South Pasadena. From the 110 Freeway, exit at Fair Oaks and head south approximately 1 mile. Turn slight right on Huntington Drive. You will find the office after about 1/2 mile on the west side of the street (cross street Maple Drive).
Can someone come to my home to help me?
Yes, we are an outreach Team that does make home visits.
Do you accept insurance?
Yes, many insurance and managed care companies will help with the
cost of treatment. We will help you to apply to your insurance company
for reimbursement; however, we do not bill insurance companies directly
unless special payment arrangements can be made. It is your responsibility
to pay your bill to The Life Adjustment Team in a timely manner.
Do you accept Medi-Cal or Medi-Care insurance?
Medi-Cal does not pay for outpatient rehabilitation, however, we
can refer you to free services and programs that do accept Medi-Cal
or Medi-Care.
How do I know if you can help me?
We are available by phone or in person, by appointment, to answer
questions about your particular situation and explain how the program
works. There is no charge for this consultation.
